ABSTRACT

Objetivo: Analizar el número de prótesis totales en relación a las parciales entregadas en el Servicio de Odontología del Hospital Nacional de Geriatría y Gerontología, en el periodo comprendido de enero 2015 a diciembre 2018". El Servicio de Odontología del HNGG tiene como prestación de servicio la confección de prótesis dentales a escala nacional. El fin de este estudio es comparar y analizar el número de prótesis totales en relación con las parciales entregadas en el Servicio de Odontología del Hospital Nacional de Geriatría y Gerontología, en el periodo comprendido de enero 2015 a diciembre 2018. Metodología: El número de prótesis estudiadas fue de 9.170. El estudio es no experimental, transeccional descriptivo, retrospectivo, donde se observan situaciones no manipulables, basándose en los registros oficiales del laboratorio y clínica dentales. Resultados: 60% de prótesis totales y 40% de prótesis parciales removibles y 50% color A2, 44% color A1 y 6% color A3. Discusión: En este centro se están realizando más prótesis totales que parciales, poniendo en evidencia el deterioro de la salud oral en la población adulta mayor. Resultados principales: Características de las prótesis realizadas por el Servicio de Odontología del Hospital Nacional de Geriatría y Gerontología fueron: 60% de prótesis totales y 40% de prótesis parciales removibles y 50% color A2, 44% color A1 y 6% color A3. Conclusiones principales: Actualmente se está realizando mayor cantidad de prótesis dentales totales removibles que prótesis parciales removibles en un promedio de 60% a 40%. Los colores más solicitados por los pacientes son los tonos blancos.


To analyze the number of total prostheses in relation to the partial ones delivered in the dentistry service of the National Hospital of Geriatrics and Gerontology, in the period of time from January 2015 to December 2018 ". The dental service of the HNGG has the service of making dental prostheses nationwide. The purpose of this study is to compare and analyze the number of total prostheses in relation to the partial ones delivered in the dentistry service of the National Hospital of Geriatrics and Gerontology, in the period of time from January 2015 to December 2018. Methodology: The number of studied prostheses were 9,170 dental prostheses. The study is non-experimental, descriptive transectional, retrospective, where non-manipulable situations are observed, based on the official records of the dental laboratory and clinic. Results: 60% of total dentures and 40% of removable partial dentures and 50% color A2, 44% color A1 and 6% color A3. Discussion: In this center, more total prostheses are being performed than partial ones, highlighting the deterioration of oral health in the older adult population. Main results Characteristics of the prostheses made by the dentistry service of the National Hospital of Geriatrics and Gerontology were: 60% of total prostheses and 40% of removable partial prostheses and 50% color A2, 44% color A1 and 6% color A3. Main conclusions Currently, more total removable dentures are being made tan removable partial dentures, an average of 60% to 40%. Being the color most requested by patients the colors of white tones.

ABSTRACT OBJECTIVE This study evaluated the oral health-related quality of life (OHRQoL) of older adults participating or not in Seniors Centers (SC). METHODS Two independent samples were compared: older adults who participate in SC (n = 124) and older adults who visited Primary Healthcare Centers (PHC) and do not participate in SC (n = 164). The data collected consisted of sociodemographic (sex, age, educational level, marital status, family income) and psychosocial characteristics—Sense of Coherence (SOC), anxiety and depression using HADS, happiness—, and oral clinical evaluation—use and need of dental prosthesis and decayed (D), missing (M), or filled (F) teeth. The resulting OHRQoL was evaluated using the Oral Health Impact Profile (OHIP-14). The Mann-Whitney test was used to assess the associations between the independent variables and the OHIP-14. Poisson regression models were also used in the analyses (α=0.05). RESULTS In the PHC, of the 270 individuals invited to participate in the study, 164 (60.7%) were interviewed and clinically examined; while in the SC, of the 166 individuals invited to participate in the study, 124 (74.7%) were interviewed and clinically examined. After adjustments for sociodemographic, psychosocial and clinical factors, we found that the impact on OHRQoL was 2.8 times higher (95%CI 2.0-4.2) for older adults who did not participate in SC. CONCLUSION Older adults who participated in SC showed better perception on OHRQoL, independently of sociodemographic, psychosocial and clinical factors.

ABSTRACT

Abstract This study aimed to explore the association between use and need of dental prostheses to the quality of life in elderly individuals in a Referral Center for Elderly Care in the northeastern of Brazil. A cross-sectional study was developed with 199 elderlies of both sexes. Data were collected from clinical examinations (WHO criteria) and a questionnaire regarding socioeconomic status, Oral Health Impact Profile index (OHIP-14) by a single calibrated examiner with Kappa = 0.91. Data were analyzed using Mann-Whitney test, Poisson regression (p<0,05). The sample consisted of 84.9% women, mean age of 70.02 (sd±6.50) years. There was an independent association between quality of life and the covariates sex and need of dental prostheses. Men showed lower values of the score (PR = 0.734, p = 0.011) than women. Individuals without normative need for dental prostheses showed lower values of OHIP-14 (PR = 0.767; p = 0.003) than those in need.


Resumo O objetivo desse estudo foi verificar a associação entre o uso e a necessidade de prótese dentária e qualidade de vida em idosos de um Centro de Referência de Atenção ao Idoso no Nordeste do Brasil. Um estudo transversal foi desenvolvido com a participação de 199 idosos de ambos os sexos. Os dados clínicos foram coletados conforme a Organização Mundial de Saúde, através de exames clínicos e questionário sobre dados socioeconômicos, índice Oral Health Impact Profile (OHIP-14), por um examinador previamente treinado (kappa=0,91). Os dados foram analisados pelo Mann-Whitney, regressão de Poisson (p>0,05). A amostra consistiu de 84,9% de mulheres, com idade média de 70,02 (dp±6,50) anos. Houve associação independente entre a qualidade de vida e as covariáveis sexo e necessidade de prótese. Idosos do sexo masculino apresentaram menores valores do escore (RP=0,734; p=0,011) do que as mulheres. Indivíduos sem necessidade normativa de prótese dentária apresentaram menores valores do OHIP-14 (RP=0,767; p=0,003) do que aqueles com necessidade.

ABSTRACT

The oral hygiene of individuals with Parkinson disease (PD) is markedly impaired by difficulties in toothbrushing due to motor impairment and tremors. Additionally, it appears that other features associated with PD have an impact on the quality of oral health. Objective: this cross-sectional observational study characterized the oral health condition of individuals with PD. Methods: fifty individuals with PD, aged 53 to 94 years, users of medication for such condition were examined. The research participants had their oral cavities examined to assess prosthetic use and need; index of Decayed, Missing and Filled Teeth (DMFT); Community Periodontal Index (CPI); Periodontal Attachment Loss (PAL); and oral mucosa. In addition, a questionnaire was applied to obtain personal data, general health, and oral health. The data were analyzed using descriptive statistics. Results: a high prosthetic use was observed in the upper arch (92%), while the lower arch revealed high prosthetic need (66%). The DMFT index showed a high number of missing teeth, CPI showed a prevalence of 43% of dental calculus, and PAL revealed 28% of attachment loss of 4-5 mm. The most frequent findings in the assessment of oral mucosa were denture stomatitis and inflammatory fibrous hyperplasia. Conclusion: PD patients refer xerostomia and present high number of missing teeth, a minority of healthy teeth, and oral lesions, representing a target population for specialized oral health care. A higiene bucal de indivíduos com doença de Parkinson (DP) é claramente prejudicada por dificuldades na escovação devido ao comprometimento motor e aos tremores. Além disso, outros aspectos relacionados à doença parecem impactar a qualidade da saúde bucal. Objetivo: este estudo observacional seccional caracterizou a condição de saúde bucal de indivíduos com DP. Métodos: foram examinados cinquenta indivíduos com DP, com idades variando entre 53 e 94 anos, que utilizavam medicamentos para DP. Os participantes da pesquisa foram submetidos a exame bucal para avaliar: uso e necessidade de prótese, índice de dentes cariados, perdidos e obturados (CPOD), Índice Periodontal Comunitário (IPC), Índice de Perda de Inserção Periodontal (PIP) e mucosa bucal. Além disso, foi aplicado um questionário para obtenção de dados pessoais e sobre saúde geral e saúde bucal. Os dados foram analisados usando estatística descritiva. Resultados: observou-se alto percentual de uso de prótese superior (92%), enquanto o arco inferior apresentou elevada necessidade de prótese (66%). O CPOD revelou um alto número de dentes perdidos; o IPC apresentou prevalência de 43% de cálculo dentário; e o PIP revelou 28% de perda de inserção de 4-5 mm. Os achados mais frequentes na avaliação da mucosa bucal foram estomatite por dentadura e hiperplasia fibrosa inflamatória. Conclusão: os indivíduos com DP mencionam xerostomia, apresentam alto número de dentes faltantes, um número reduzido de dentes saudáveis e lesões bucais, representando uma população-alvo para o cuidado de saúde bucal especializado. ABSTRACT

Resumo O objetivo deste estudo foi estimar a prevalência de dificuldade na mastigação e fatores associados em adultos de 20 a 59 anos em Patos, PB, Nordeste, Brasil. Foi realizado um estudo transversal com uma amostra aleatória de 532 indivíduos. A dificuldade na mastigação foi avaliada por meio de pergunta sobre dificuldade causada por problemas com dentes ou dentadura. Foram realizadas análises descritiva, bivariada e multivariada por meio de regressão de Poisson. A prevalência de dificuldade na mastigação foi de 30,5%. Na análise multivariada, os fatores associados à dificuldade na mastigação foram: faixa etária, escolaridade, tempo desde a última consulta, perda dentária severa, ausência de dentição funcional, uso e necessidade de prótese dentária, dor de origem dental e sinais e sintomas de alterações na ATM. A magnitude das associações entre as variáveis, com destaque para perda dentária e necessidade de prótese dentária, reforça a importância deste indicador subjetivo na avaliação da condição de saúde bucal dos indivíduos adultos e mostra que a dificuldade na mastigação está associada a uma estrutura multidimensional de fatores.


Abstract The aim of this study was to estimate the prevalence of chewing difficulty and associated factors in adults aged 20 to 59 years in Patos, in the State of Paraiba in the Northeast Region of Brazil. A cross-sectional study was conducted with a random sample of 532 participants. The outcome chewing impairment was assessed using the question "How often do you have difficulty eating due to problems with your teeth or dentures?". Crude and adjusted prevalence ratios were estimated using Poisson regression. The prevalence of chewing difficulty was 30.5%. An association remained between the following variables and chewing difficulty in the final multivariate models: age group, schooling, being a smoker/non-smoker, length of time since last visit to the dentist, severe tooth loss, absence of functional dentitions, dental prosthesis use, need for a dental prosthesis, oral pain, and signs and symptoms of TMJ dysfunction. The magnitude of the associations between the variables and chewing difficulty, notably tooth loss and the need for a dental prosthesis, emphasize the importance of subjective indicators for assessing the oral health status of adults and shows that chewing difficulty is associated with a range of multi-dimensional factors.

RESUMO Objetivo Avaliar a autopercepção das condições alimentares de idosos usuários de prótese dentária, verificando sua interferência na alimentação. Método Participaram 60 idosos com idade entre 60 e 88 anos, usuários de prótese dentária total ou parcial, que frequentam assiduamente o Centro de Convivência Nair Ventorin Gurgacz - FAG em Cascavel, Paraná. Todos os indivíduos responderam a um questionário de identificação e ao protocolo Índice de Determinação da Saúde Bucal Geriátrica (GOHAI). Resultados Os idosos do estudo, maioria mulheres e usuários de prótese removível bimaxilar com mais de 30 anos, classificam sua alimentação como "boa" e não possuem preferência por consistência alimentar, embora frequentemente sintam dor ao mastigar os alimentos. A média pontuada no protocolo GOHAI foi classificada como "ruim" e o maior prejuízo dos idosos ocorreu no domínio físico, que engloba questões relacionadas à mastigação, deglutição e fala. Não foram constatadas relações entre o tempo de utilização da prótese dentária com a idade e os escores do GOHAI. Conclusão Embora os idosos raramente tenham queixa sobre o modo de se alimentar, referindo desconforto ou constrangimento, apresentam uma média abaixo do esperado no protocolo, indicando que, ainda que não relatem, muitas modificações podem estar acontecendo, gradativamente, e que soam como naturais, gerando prejuízos à qualidade de vida em alimentação do idoso.


ABSTRACT Purpose To evaluate the self-perception of the food conditions of elderly users of dental prosthesis, in order to verify the interference that dental prosthesis has in their feeding. Methods The present study counted with 60 elderly, participants of the Centro de Convivência Nair Ventorin Gurgacz (Community Center), aged between 60 and 88 years old. All participants answered to a questionnaire elaborated by the researchers and the Geriatric Oral Health Assessment Index (GOHAI). Results The elderly who participated in this study, mostly women, used a bimaxillary removable prosthesis for over 30 years; although they classified their nutrition as "good" and did not present preference for specific food consistency, they frequently felt pain while chewing. The average score of the GOHAI was 29.73, considered "low", the index dimension with worst score was physical function, that includes eating, speaking and swallowing. No relation was observed between time of use of dental prosthesis, age and the GOHAI scores. Conclusion The elderly rarely complaint about discomfort or embarrassment on feeding, however, they present an average lower than expected in the GOHAI index, which indicates that, although they do not report it, many deteriorations, that seem to be natural may be happening and may be the cause of worst quality of life.

ABSTRACT

Abstract Introduction: Adequate nutrition, including intake of dietary calcium and vitamin D, is important to maintain bone health. Evidence suggests that a deficiency in micronutrients may contribute to bone loss during aging and exert generalized effects on chronic inflammation. Recently, the Dietary Inflammatory Index (DII) was developed to assess the inflammatory potential of individual diets. Our aim was to evaluate the DII in a representative sample and verify its association with low-impact fractures. Methods: Individuals from The Brazilian Osteoporosis Study (BRAZOS) database had their DII calculated. BRAZOS is an important cross-sectional epidemiological study carried out with a representative sample of men and women ≥40 years old. The research was conducted through in-home interviews administered by a trained team. Nutrition Database System for Research (NDSR) software was used to analyze data on the intake of nutrients, which were employed to calculate the DII using Statistical Analysis Software (SAS®) and Statistical Package for the Social Sciences (SPSS®) to assess its association with low-impact fractures. Results: A total of 2269 subjects had their DII score calculated using information from 24-h recall data. Males had lower DII than females (DII = 1.12 ± 1.04 vs DII = 1.24 ± 0.99, p = 0.012). Women taking statins had lower DII (DII = 0.65 ±1.14 vs DII + 1.26 ± 0.98, p = 0.002), indicating a greater potential for diet-related anti-inflammatory effects. Conclusion: Our findings suggest that women might have a pro-inflammatory diet pattern compared to men. However, we did not find any association between DII scores and low-impact fractures.

Resumo Este estudo teve como objetivos estimar a prevalência da necessidade de prótese e tratamento dental em idosos brasileiros, e verificar a associação entre essas necessidades e fatores contextuais e individuais. Realizou-se estudo com dados (n = 7.619) da Pesquisa Nacional de Saúde Bucal - SB Brasil 2010. Modelos de regressão logística multinível foram utilizados para estimar odds ratio e intervalos de 95% de confiança entre as necessidades de tratamento e as variáveis contextuais (Índice de Desenvolvimento Humano Municipal, Coeficiente de Gini e cobertura de saúde bucal na Estratégia de Saúde da Família) e individuais (sexo, renda, escolaridade e cor da pele). Para a necessidade de tratamento dental, ser homem, apresentar menor escolaridade, menor renda, cor da pele autorrelatada como não branca e residir em municípios com menor IDH -M foram fatores que aumentaram as chances de os indivíduos necessitarem de tratamento; para a necessidade de prótese, a cor da pele autorreferida como não branca, menor renda e menor escolaridade também aumentaram as chances dos idosos necessitarem de prótese. Os resultados indicaram associação entre as necessidades de tratamento e determinantes individuais e contextuais.


Abstract The aims of this study were to estimate the prevalence of prosthetic requirements and dental treatment needs among the elderly Brazilian population and the associations with contextual and individual determinants. Data were taken from the Brazilian Oral Health Survey (SB Brasil 2010) and a sample of 7,619 Brazilian people aged 65-74 years were investigated. Multilevel logistic regression models were used to estimate the odds ratio (OR) and 95% confidence intervals between treatment needs and contextual variables (Municipal Human Development Index, Gini coefficient and oral health coverage in the Family Health Strategy) and individual (gender, income, education and skin color). The prevalence of treatment need ranged from municipalities (p < 0.05). Of the individual variables, gender was associated with dental treatment need (OR = 1.18, 95% CI 1.05 to 1.31); lower education and non-white skin were risk factors (OR = 1.18; 95% CI 1.05 to 1.31 and OR = 1.28, 95% CI 1.15 to 1.43); the non-white skin color represented a risk factor (OR = 1.83; 95% CI 1,38- 2,42) and for the need for prosthesis income was a protection factor (OR = 0.39; 95% CI 0, 30-.51). The results showed an association between the individual factors and treatment needs.

RESUMO: Introdução: O uso de prótese e a perda dentária em idosos estão associados a impactos significativos na saúde geral e na qualidade de vida. A avaliação contínua do perfil de saúde bucal dessa população é importante para o planejamento das ações e políticas da área. Objetivo: Foram objetivos deste trabalho avaliar a prevalência de perda dentária e uso de próteses entre idosos em diferentes períodos, verificar as características sociodemográficas associadas à dentição funcional (mais de 20 dentes) e avaliar o impacto do uso de prótese e da perda dentária na autoavaliação da saúde bucal. Métodos: A amostra foi composta por pessoas de 60 anos ou mais, participantes do Estudo Saúde, Bem-Estar e Envelhecimento (SABE). A avaliação da prevalência de perda dental e uso de próteses foi feita a partir da comparação dos dados coletados nos anos de 2000, 2006 e 2010. As análises dos fatores associados à dentição funcional e autoavaliação de saúde bucal foram realizadas com base nos dados coletados no ano de 2010. A comparação do perfil de saúde bucal ao longo dos três períodos foi feita por meio de análises descritivas e comparação de intervalos de confiança. Realizou-se análise de regressão logística múltipla para avaliar os fatores associados à dentição funcional e à autoavaliação da saúde bucal. Resultados: A prevalência de perda dental e uso de próteses se manteve constante ao longo dos três anos analisados. A dentição funcional foi significativamente associada à escolaridade, sexo e cor/gênero. Idosos com necessidade de próteses e bolsa periodontal apresentaram mais chances de autoavaliação ruim. Conclusão: Não houve redução da prevalência de perda dentária e uso de próteses ao longo de dez anos na população estudada. A dentição funcional está associada a desigualdades sociodemográficas. A autoavaliação de saúde bucal está associada à necessidade de próteses.


ABSTRACT: Introduction: The use of dental prosthesis and the tooth loss in elderly people are associated with significant impact on the overall health and quality of life. Continuous assessment of oral health profile in this population is important for planning the actions and policies of the area. Objectives: The aims of this study were to assess the prevalence of tooth loss and use of dental prosthesis among the elderly people in different periods, to evaluate the association between functional dentition (20 teeth or more) and socioeconomic factors, and to evaluate the impact of tooth loss and use of dental prosthesis on self-rated oral health. Methods: Thesample consisted of people aged 60 years and older who participated in the Health, Well-Being, and Aging Study (SABE). Data from the years 2000, 2006, and 2010 were used to assess the prevalence of tooth loss and use of dental prosthesis. Analysis of the factors associated with the functional dentition and self-rated oral health was based on the data collected in 2010. Comparison of oral health profile over the 3 years was done through descriptive analysis and comparison of confidence intervals. Multiple logistic regression models were used to assess the factors associated with functional dentition and self-rated oral health. Results: The prevalence of tooth loss and use of dental prosthesis remained constant over the three periods analyzed. Functional dentition was significantly associated with education, sex, and race/gender. Individuals in need of dental prosthesis and with periodontal pocket were more likely to report poor oral health. Conclusion: There was no reduction in the prevalence of tooth loss and in the use of dental prosthesis over 10 years. Functional dentition is associated with socioeconomic inequalities. Self-rated oral health is associated with the need of dental prosthesis.

ABSTRACT

ABSTRACT OBJECTIVE To investigate the association between life course socioeconomic conditions and two oral health outcomes (edentulism and use of dental prostheses among individuals with severe tooth loss) among older Brazilian adults. METHODS This was a cross-sectional study with data from the Brazilian Longitudinal Study of Aging (ELSI-Brazil) which includes information on persons aged 50 years or older residing in 70 municipalities across the five great Brazilian regions. Regression models using life history information were used to investigate the relation between childhood (parental education) and adulthood (own education and wealth) socioeconomic circumstances and edentulism and use of dental prostheses. Slope index of inequality and relative index of inequality for edentulism and use of dental prostheses assessed socioeconomic inequalities in both outcomes. RESULTS Approximately 28.8% of the individuals were edentulous and among those with severe tooth loss 80% used dental prostheses. Significant absolute and relative inequalities were found for edentulism and use of dental prostheses. The magnitude of edentulism was higher among individuals with lower levels of socioeconomic position during childhood, irrespective of their current socioeconomic position. Absolute and relative inequalities related to the use of dental prostheses were not related to childhood socioeconomic position. CONCLUSIONS These findings substantiate the association between life course socioeconomic circumstances and oral health in older adulthood, although use of dental prostheses was not related to childhood socioeconomic position. The study also highlights the long-lasting relation between childhood socioeconomic inequalities and oral health through the life course.

ABSTRACT Objective To evaluate the oral health of patients with head and neck cancer after antineoplastic treatment, and to compare them with patients with no history of cancer. Methods A total of 75 patients, divided into Study Group, composed of individuals after antineoplastic treatment (n=30), and Control Group, with individuals with no history of cancer (n=45), aged 37 to 79 years. The oral health status was evaluated through the index of decayed, missing or filled permanent teeth (DMFT), community periodontal index and evaluation of the use and need of prosthesis. All of these items were evaluated according to the criteria recommended by the World Health Organization. The statistical analysis was descriptive and used the Pearson's χ2 test. Results The community periodontal index was higher in the Study Group when compared to the Control Group (p<0.0001). The need for an upper (p<0.001) and lower (p<0.0001) prostheses was higher in the Study Group. Also, the use of upper prosthesis was higher in the Study Group (p<0.002). The missing or filled permanent teeth index between the two groups (p>0.0506) and the use of lower prosthesis (p>0.214) did not present a relevant statistical difference. Conclusion Periodontal disease and edentulism are the most significant changes in individuals who received antineoplastic therapy for head and neck cancer as well as greater need for oral rehabilitation.


RESUMO Objetivo Avaliar a saúde bucal de pacientes com câncer de cabeça e pescoço após tratamento antineoplásico, e compará-los com pacientes sem histórico de câncer. Métodos Foram avaliados 75 pacientes, divididos em Grupo de Estudo de indivíduos após tratamento antineoplásico (n=30) e Grupo de Controle de indivíduos sem histórico de câncer (n=45), com faixa etária de 35 a 79 anos. A condição de saúde oral foi avaliada pelo índice de dentes permanentes cariados, perdidos e obturados (CPOD), pelo índice periodontal comunitário e por uma avaliação de uso e necessidade de prótese, conforme critérios preconizados pela Organização Mundial da Saúde. A análise estatística foi descritiva e realizada por meio do teste do χ2 de Pearson. Resultados O índice periodontal comunitário foi maior no Grupo de Estudo quando comparado ao controle (p<0,0001). A necessidade de prótese superior (p<0,001) e inferior (p<0,0001) foi maior no Grupo de Estudo. O uso de prótese superior foi maior no Grupo de Estudo (p<0,002). O índice de dentes permanentes cariados, perdidos e obturados entre os dois grupos (p>0,0506) e o uso de prótese inferior (p>0,214) não apresentaram diferença estatística relevante. Conclusão A doença periodontal e o edentulismo são as alterações mais significativas em indivíduos que receberam terapia antineoplásica de câncer de cabeça e pescoço, assim como maior necessidade de reabilitação oral.

OBJECTIVE: The objective of this study was to explore the factors associated with inadequate chewing in an adult and elderly population of a city in the southern region of Brazil. METHODS: This was a cross-sectional study based on a population home-based inquiry (DCH-POP) in southern Brazil. Individuals were interviewed by trained interviewers to create a standardized procedure. In a pilot study, the Questionnaire of Human Communication Disorders (DCH-POP) was created and validated to identify self-reported speech and language, swallowing and hearing disorders. The outcome was dichotomized into either having adequate chewing or not, as assessed by a series of questions about chewing ability. Analyses of absolute and relative frequencies were measured according to the studied variables. A Poisson regression was applied at a significance level of 5%. RESULTS: A total of 1,246 people were interviewed. Inadequate chewing was found in 52 (5.6%) individuals, with a higher prevalence in the elderly (11.8%) than in adults (5.2%). In the final model, the following factors were associated with inadequate chewing: being 61 years of age or older (prevalence ratio or PR=9.03; 95% CI: 1.20-67.91), loss of teeth and use of unadjusted prosthesis (PR=3.50; 95% CI: 1.54-7.95), preference for foods of soft consistency (PR=9.34; 95% CI:4.66-8.70) and difficulty in nasal breathing (PR=2.82; 95% CI: 1.31-6.06). CONCLUSION: Age, oral health status through dental prosthesis, preference for foods of soft consistency and difficulty breathing through the nose were factors associated with chewing inability in adults and the elderly.

Resumo: O objetivo deste estudo foi avaliar o uso e a necessidade de prótese dentária entre os idosos brasileiros (65-74 anos) e verificar fatores associados. Foram analisados dados de 7.496 idosos participantes do Pesquisa Nacional de Saúde Bucal realizado em 2010 (SBBrasil, 2010). O uso e a necessidade de prótese dentária foram usados como desfechos. As variáveis de exposição incluíram características demográficas, socioeconômicas, de uso de serviços e de autopercepção da saúde bucal. Análises descritiva, bivariada e multivariada foram realizadas. A prevalência de uso e necessidade de prótese dentária foi de 78,2% e 68,7%, respectivamente. A Região Nordeste foi a que apresentou a menor prevalência de uso (71,3%) e a maior de necessidade prótese dentária (82,9%). As análises multivariadas por meio de regressão de Poisson revelaram maior uso de prótese dentária em mulheres, naqueles com 5 a 7 anos de estudos e nos que foram a serviço particular, e houve menor uso em indivíduos pretos e com necessidade autorreferida de prótese dentária. Menor necessidade de prótese dentária foi observada em mulheres e nos usuários de serviço particular, e maior naqueles que autorreferiram necessidade de prótese dentária. Diferenças regionais foram observadas na distribuição do uso e da necessidade de prótese dentária. Ainda assim, os achados revelaram altas prevalências de ambos os desfechos em todas as regiões. Variáveis socioeconômicas, demográficas e de uso de serviços influenciaram a ocorrência de uso e de necessidade de prótese dentária.


Abstract: The objectives of this study were to evaluate the use of and need for dental prostheses among elderly Brazilians (65-74 years of age) and to verify associated factors. Data were analyzed from 7,496 elderly participants in the National Oral Health Survey in 2010 (SBBrazil 2010). Use of and need for dental prosthesis were the outcomes. The exposure variables included demographic and socioeconomic characteristics, dental services use, and self-rated oral health. Descriptive, bivariate, and multivariate analyses were performed. Prevalence rates for use of and need for dental prostheses were 78.2% and 68.7%, respectively. The Southeast Region had the highest prevalence of use (71.3%) and the greatest need for dental prostheses (82.9%). Multivariate Poisson regression analyses showed greater use of dental prostheses by women, individuals with 5 to 7 years of schooling, and users of private dental services, and lower use by black individuals and those with self-reported need for dental prostheses. Less need for dental prostheses was seen in women and in users of private services, and greater need in those with self-reported need for dental prostheses. Regional differences were seen in the distribution of use and need for dental prostheses. Still, the findings showed high prevalence rates for both outcomes in all regions of Brazil. Socioeconomic and demographic variables and use of dental services influenced the use of and need for dental prostheses.


Resumen: El objetivo de este estudio fue evaluar el uso y la necesidad de prótesis dentales entre los ancianos brasileiros (65-74 años) y verificar sus factores asociados. Se analizaron datos de 7.496 ancianos participantes en la Encuesta Nacional de Salud Bucal, realizada en 2010 (SBBrasil 2010). El uso y la necesidad de prétesis dentales se usaron como resultados. Las variables de exposición incluyeron características demográficas, socioeconómicas, de uso de servicios y de autopercepción de salud bucal. Se realizaron análisis descriptivos, bivariados y multivariados. La prevalencia de uso y necesidad de prótesis dentales fue de un 78,2% y 68,7%, respectivamente. La región nordeste fue la que presentó la menor prevalencia de uso (71,3%) y la mayor de necesidad prótesis dentales (82,9%). Los análisis multivariados, mediante la regresión de Poisson, revelaron un mayor uso de prótesis dentales en mujeres, en aquellos con 5 a 7 años de estudios y en los que fueron a servicios privados, y hubo un menor uso en individuos negros y con necesidad autorreferida de prótesis dentales. Se observó una menor necesidad de prótesis dentales en mujeres y en los usuarios de servicios particulares, y mayor en aquellos que autoinformaron una necesidad de prótesis dentales. Se observaron diferencias regionales en la distribución del uso y necesidad de prótesis dentales. Incluso así, los hallazgos revelaron altas prevalencias de ambos en los resultados en todas las regiones. Variables socioeconómicas, demográficas y de uso de servicios influenciaron la ocurrencia de uso y necesidad de prótesis dentales.

Resumo O presente estudo caracterizou a condição de saúde bucal e os principais motivos autorrelatados da extração dentária em uma população de adultos. Estudo transversal que examinou 248 adultos de 20-64 anos, representativos da população de Piracicaba (SP). O exame bucal domiciliar utilizou os índices CPOD, CPI, uso e necessidade de prótese dentária segundo critérios da OMS e presença de biofilme visível. Foram coletados dados demográficos, socioeconômicos e motivos da extração dentária por meio de questionário. A análise descritiva foi estratificada pela idade em 20-44 e 45-64 anos. O CPOD médio foi 20,37, P = 3,34 nos adultos jovens e P = 13,41 nos mais velhos. Bolsa periodontal (CPI ≥ 3) foi encontrada em 20,5% dos adultos jovens e 53,0% dos mais velhos. Enquanto 38,8% usavam prótese superior, 46,7% necessitavam de prótese inferior. A dor foi o motivo autorrelatado mais prevalente para realização da extração dentária, sendo esta escolha principalmente pela falta de outra opção de tratamento e no serviço privado. Conclui-se que os adultos jovens (20-44 anos) apresentaram menos dentes perdidos e doença periodontal, uso e necessidade de prótese. Dor e falta de opção de outros tratamentos foram os principais motivos autorrelatados para realização das extrações dentárias.


Abstract The study characterized the oral health condition and main self-reported reasons for tooth extraction in an adult population. The cross-sectional study examined 248 adults aged 20-64 years, representative of the population of Piracicaba, São Paulo, Brazil. The oral examination conducted in households used the DMFT and CPI indexes, use and necessity of prosthodontics according to the WHO criteria and the presence of visible biofilm. Demographic and socioeconomic data were collected along with reasons for tooth extraction through a questionnaire. Descriptive analysis was stratified by age in groups: 20-44 and 45-64 years old. The average DMFT was 20.37 (EP = 0.50), P = 3.34 (EP = 0.33) for young adults and P = 13.41 (EP = 1.45) for the older adults. Gingival pockets (CPI ≥ 3) were found on 20.5% of young adults and 53.0% of the older ones. While 38.8% used upper prosthesis, 46.7% needed lower prosthesis. Pain was the most prevalent self-reported reason for tooth extraction (37.5%), being this choice primarily because of lack of another treatment option (52%) and done in the private sector (47.2%). We concluded that young adults (20-44 years old) showed less missing teeth, periodontal diseases, and need for prosthetic use. Pain and lack of options of other treatments were the main self-reported reasons for performing tooth extractions.

Introdução: Candida albicans é um fungo que se destaca pela alta frequência de colonização e infecção no hospedeiro humano. É comumente encontrado na cavidade bucal e pode causar infecção em indivíduos que se encontram com deficiência do sistema imunológico ou em portadores de próteses removíveis. Objetivo: realização de um estudo epidemiológico referente à prevalência de candidose bucal em pacientes hospitalizados, avaliando possíveis fatores de risco, como: diabetes, presença de neoplasias malignas e utilização ou não de algum tipo de prótese dentária removível. Metodologia: foram analisados 141 pacientes internados no Hospital da Cidade (HC) no município de Passo Fundo/RS, a fim de avaliar a presença ou ausência de candidose bucal, bem como alguns fatores de risco relacionados a tal enfermidade. Foi feito o exame clínico e, quando possível, realizada a documentação fotográfica dos casos. Os dados coletados foram tabulados em planilha eletrônica e analisados por meio de estatística descritiva de frequência e teste Qui-quadrado ao nível de significância de 5%. Resultados: observou-se que os fatores sistêmicos não apresentaram significância com a presença de candidose (p = 0,726), porém o uso de próteses removíveis (p = 0,042) e o gênero (p = 0,05) apresentaram significância estatística. Conclusão: Na amostra estudada independente de alguns pacientes internados apresentarem doenças sistêmicas (diabetes e/ou neoplasia maligna), não foi notada relação destas com a presença de candidose bucal. Contudo, notou-se que a utilização de próteses dentárias removíveis e o gênero feminino constituem-se como fatores preditivos para a ocorrência de candidose bucal.


Introduction: Candida albicans is a fungus that stands out for the high frequency of colonization and infection in the human host. It is commonly found in the oral cavity and can cause infection in individuals who are disabled or the immune system in patients with removable dentures. Objective: to conduct an epidemiological study regarding the prevalence of oral candidiasis in hospitalized patients, assessing possible risk factors such as diabetes, presence of malignant neoplasms and use or not some kind of removable dental prosthesis. Methods: we analyzed 141 patients admitted to City Hospital (HC) in the city of Passo Fundo/RS, to assess the presence or absence of oral candidiasis, as well as some risk factors related to this disease. Clinical examination was made and, where possible, carried out photographic documentation of cases. Data were tabulated in a spreadsheet and analyzed using descriptive statistics and frequency chi-square test at the 5% significance level. Results: it was observed that the systemic factors were not significant with the presence of candidiasis (p = 0.726), but the use of removable dentures (p = 0.042) and gender (p = 0.05) were considered significant. Conclusion: in the sample studied independent of some hospitalized patients had systemic diseases (diabetes and/or malignancy), was noted their relation with the presence of oral candidiasis. However, it was noted that the use of removable dentures and females constitute as predictors for the occurrence of oral candidiasis (AU)

Abstract The aim of this study was to investigate the association between oral health status and central obesity (CO) in Brazilian independent-living elderly. A cross-sectional study was carried out in a sample of 489 elderly, who were participants of the Study on Aging and Longevity, in Londrina, state of Parana. The number of natural teeth and use of prostheses were evaluated according to the World Health Organization criteria. The presence of CO was assessed using measures of waist circumference (WC) and waist-hip ratio (WHR). Information concerning sociodemographic profile and some systemic conditions was also collected. Data were analyzed using stepwise logistic regression, α=5%. According to WC and WHR measures, the prevalence of central obesity was 79.3% and 76.1%, respectively. CO according to WC was not associated with oral status. Considering the WHR measure, the following oral conditions were associated to CO: having fewer natural teeth (OR = 2.61; 95%CI = 1.17-5.80), being edentulous and wearing both upper and lower complete dentures (OR = 2.34; 95%CI = 1.11-4.93), and being edentulous wearing only the upper complete denture (OR = 2.64; 95%CI = 1.01-6.95). Traditional risk factors for CO such as gender, dyslipidemia, hypertension and diabetes were associated with both measures. A poor oral health due to extensive tooth loss, whether partial or complete, even if rehabilitated by removable prostheses, may be considered a good predictor of CO in Brazilian independent-living elderly.

Abstract The purpose of this study was to investigate the relationship between Sense of Coherence (SOC) and oral health clinical variables (number of teeth present and absence of need for dental prostheses). The sample consisted of 720 adults and elderly Brazilians. The data were collected at home using the SOC-13 scale - a form of clinical examination for the evaluation of oral conditions - and a questionnaire evaluating socioeconomic aspects and the use of dental services. Statistical analysis, both univariate and multivariate, was performed by Poisson regression with robust variance adjustment. The average age of the participants was 60.2 years, and they were predominantly female (57.8%). Among the participants who had a strong SOC, the absence of the need for dental prostheses was 34% higher than among those with a weak SOC, demonstrating a significant difference between the groups (PR = 1.34, 95%CI = 1.06-1.70; p = 0.015). Individuals who had a strong SOC had a 5% higher prevalence of 14 or more existing teeth than those with a weak SOC, which was statistically significant after adjustment for co-factors (PR = 1.05, 95%CI = 1.01-1.11, p = 0.033). A strong personal Sense of Coherence has a beneficial influence on the oral health of adults and older people in Brazil.

INTRODUÇÃO: O processo de envelhecimento populacional é crescente em nosso país e representa um desafio tanto à sociedade como aos sistemas de saúde. Diante disso, torna-se necessário o planejamento de políticas públicas voltadas à prevenção e ao controle das doenças bucais tendo em vista que a presença destas patologias afeta diretamente a qualidade de vida do idoso. Neste contexto, é importante que se tenha conhecimento dos dados epidemiológicos deste grupo populacional. OBJETIVO: avaliar a condição de saúde bucal de idosos institucionalizados em Teresina ­ PI. METODOLOGIA: coleta de dados foi realizada em duas instituições de acolhimento ao idoso, localizadas em Teresina-PI, onde foram analisados CPO-D, Índice Periodontal Comunitário (IPC) e uso e/ou necessidade de prótese de 19 idosos, institucionalizados, com idade entre 65-74 anos. RESULTADOS E DISCUSSÃO: foi encontrado um elevado índice de CPO-D (29,95), presença de 8,77% dos sextantes com doenças periodontais, altas porcentagens de idosos edêntulos, onde 50% usavam prótese na arcada superior e 23,82% na inferior. Quanto à necessidade do uso de prótese nas arcadas, apresentaram-se os seguintes valores: 52,64% na arcada superior e 84,21% na inferior. Os resultados demonstram que as condições de saúde bucal nos idosos institucionalizados examinados são precárias. CONCLUSÃO: medidas de promoção e recuperação da saúde são necessárias nas instituições públicas e filantrópicas de longa permanência em Teresina-PI (AU)


Introduction: Population aging in our country represents a challenge both for society and healthcare systems. Therefore, it becomes necessary the development of public policies for the prevention and control of oral diseases in the elderly given that the presence of these conditions directly affect the quality of life of that segment of the population. AIM: to evaluate the oral health status of institutionalized elderly in Teresina, State of Piauí. Metodology: Data collection was conducted in two long permanence institutions for the elderly, located in Teresina, Piauí. Thus, 19 institutionalized volunteers, between the ages of 65 and 74 years, had their oral conditions analyzed through the analysis of three indexes: DMF Index, Community Periodontal Index (CPI) and the use or need for dental prosthesis. Results and discussion: It was found a high DMF Index (29.95), a presence of 8.77% of sextants with periodontal disease, high percentages of edentulous ones, in which 50% used prosthesis in the upper arch and 23.82% in the lower arch. The results show that the oral health status in the elderly are not satisfactory. Conclusion: oral health promotion measures are needed in public and philanthropic long-permanence institutions in Teresina, Piauí (AU)

Objetivando estudar os serviços de reabilitação protética da rede de saúde pública, buscou-se dados sobre a implantação e manutenção desses serviços no estado de Minas Gerais, e sobre o perfil dos municípios que credenciaram laboratórios regionais de próteses dentárias (LRPD) e dos que não credenciaram. Para o estudo, tomou-se como referência três grupos de municípios: grupo I representado pelos municípios que não credenciaram LRPD (68% dos municípios do estado), grupo II municípios que perderam o credenciamento (6%) e o grupo III representado pelos municípios que possuem LRPD credenciados (26%). ABSTRACT

El objetivo de este estudio fue medir el impacto del edentulismo y el uso de prótesis dentales en la calidad de vida de los adultos mayores en Bogotá, Colombia. El edentulismo es frecuente en los adultos mayores y afecta profundamente su calidad de vida. Actualmente existen pocos datos epidemiológicos disponibles sobre este tema en nuestro medio. Los datos fueron extraídos del estudio SABE-Bogotá, estudio transversal por conglomerados llevado a cabo en el 2012. Se uso el EQ-VAS (escala visual análoga) como instrumento de medición de la percepción de la calidad de vida. El estudio incluyó 2.000 individuos mayores de 60 años. Se usó el test de Sperman-Rho para analizar la correlación entre el EQ-VAS, el edentulismo y el uso de prótesis. Las pruebas de Chi-cuadrado, ANOVA y t-test se usaron para estudiar las diferencias en los puntajes del EQ-VAS entre los sujetos edéntulos y sanos. La significancia estadística se estableció en <0.05. De los 2000 encuestados, 98.3 por ciento fueron edéntulos, 73.0 por ciento reportaron perdida de mas de la mitad de sus dientes, 76.9 por ciento reportaron usar prótesis dentales y 23.7 por ciento problemas relacionados con la alimentación. La edad avanzada, el estrato social bajo y el bajo nivel educativo se relacionaron con mayor pérdida dental. Individuos con pocos dientes y prótesis dentales tuvieron puntajes bajos en el EQ-VAS (p=0.22). La presencia de edentulismo también mostró una correlación negativa con los puntajes del EQ-VAS (rho= -0.102, p<0.01). Se concluye que EQ-VAS es un instrumento de gran utilidad para la medición de calidad de vida en contextos relacionados a salud oral. El edentulismo afecta significativamente la calidad de vida en los adultos mayores y el uso de prótesis dentales no mejora la percepción de la calidad de vida.


The objective of this study was to measure the impact of edentulismand dental prostheses on quality of life (QOL) in older adults inBogotá, Colombia. Edentulism is a frequent condition in olderadults and has great impact on their QOL. No epidemiologicaldata are currently available on edentulism among older adults inColombia. Data were obtained from the SABE-Bogotá study, across-sectional study conducted in 2012, and used to analyze theEQ-VAS (Visual Analog Scale) from the EuroQol instrument tomeasure the perception of quality of life (QOL) in relation toedentulism. The study included 2,000 individuals over 60 yearsold. The Spearman-Rho correlation was used to analyze thecorrelation between EQ-VAS and edentulism. Chi-Square, ANOVAand t-test were used to study the differences in EQ-VAS scoresbetween edentulous and healthy subjects. Statistical significancewas set at p<0.05. Of the 2000 respondents, 98.3% wereedentulous, 73.0% reported half or more missing teeth, 76.9%used dental prostheses and 23.7% had related eating problems.Older age, lower social class and lower education were related toedentulism. Individuals with fewer teeth and dental prostheses hadlower EQ-VAS scores (p<0.05) and dental prosthesis did notimprove EQ-VAS scores (p=0.22). Edentulism also showed asignificant negative correlation with EQ-VAS scores (rho= -0.102,p<0.01). In summary, EQ-VAS is a useful tool for measuring theperception of QOL in dental health scenarios. Edentulismsignificantly affects QOL in older adults and the use of dentalprosthesis does not improve the perception of QOL.
